I tender herewith the sum of R560.00 being the subscription fee for the current year. I fully understand that if I am not elected a member of the club, the sum aforesaid will be refunded to me. I certify that the information submitted is correct in every respect and that I will abide by the Constitution, bye-laws and rulings of the Club or its Committee, which Constitution I undertake to have read immediately membership hereof has been confirmed. I specifically acknowledge that I may only resign my and/or my family membership in writing. Upon resignation I shall be obliged to pay all outstanding liabilities to the club. Should I leave the club without resigning in writing, I shall continue to be liable for subscription fees. I hereby agree that in return for allowing me to be a member of the Cheetahs Athletic Club and to take part in its activities and/or competitions, I waive any claim which I may in the future have against the Club, its Agents or Representatives for any damage, injury or loss sustained by myself or any members of my family, while taking part in the Club's activities. Please note that membership application forms will not be processed until full payment is made. No refunds will be given after acceptance as a member, for whatsoever reason. I acknowledge that pending the election decision I shall, for a maximum period of 3 months, enjoy a probationary membership status. I shall however, not be entitled to vote or sign on visitors during this period.
